Healthcare systems around the globe are challenged to deliver an effective chronic disease management due to the complex nature and multifactorial causes of chronic diseases. Evidence has shown that health care can be delivered more effectively and efficiently through a multidisciplinary team approach, in which providers from different disciplines are supported with the necessary resources and expertise to better manage their patients with chronic diseases. With a diverse group of healthcare professionals on board, the team can ensure their patients' treatment goals are attained and maintained. This workshop will illustrate a collaborative healthcare model which delivers a systematic chronic disease management for the patients in the primary care setting.
